Please fill out this form completely and mail it plus the transcript fee, to:

Please allow no less than 2-3 weeks from the time of our receipt of your request to the time of arrival of your transcript(s).

Please Note – Most Studio A.C.T. transcripts are pre-digital hard copies, stored off-site in a secure storage facility, requiring physical transfer from there to the A.C.T. Conservatory offices in San Francisco. For this reason, we are unable to guarantee rush requests. Please allow no less than four (4) weeks from the time of our receipt of your request to the time of your transcript(s) arriving at their destination.

Transcript Fees

First Copy - $5; each additional copy requested with this form is $4. No transcript request will be processed without the accompanying fee. Check or money order only. The release of A.C.T. transcripts is contingent upon the fulfillment of all tuition/fee and library obligations.
